The United Rural Health & Wellness Initiative of North Carolina
PURPOSE
To provide holistic, outcome-based services customized to address poverty and identified social determinants of health in rural NC counties.
STRATEGY:
Utilizing the Results Oriented Management & Accountability Model (ROMA) for program development & design, the URWell(NC) Initiative provides solutions to the most pressing concerns identified in each county's four-year community health assessment. It also supports the Healthy North Carolina Initiative and addresses the impact of COVID-19 pandemic.

This state-of-the-art approach to holistic care combines personal care, medical services and assistance with social resources provided through one primary care facility. This model is most effective in highly marginalized, rural communities for which transportation, continuums of care, and personal accountability are barriers to holistic health.
